Immunodeficiency refers to conditions in which the immune system does not function optimally, resulting in increased susceptibility to infections. These conditions may be primary (due to an underlying immune system defect) or secondary to other medical conditions or treatments.

Symptoms and presentation

Patients with immunodeficiency often present with recurrent, severe or unusually persistent infections. These may include recurrent sinus infections, chest infections, pneumonia, ear infections, or infections that require repeated or prolonged courses of antibiotics. Some patients may experience infections that are more severe than expected, occur at an unusually young age, or are caused by uncommon organisms. Others may have a history of poor response to standard treatments or slow recovery from infections. In some cases, immunodeficiency may be suspected due to abnormal blood test results, such as low immunoglobulin levels or impaired vaccine antibody responses.

Causes of immunodeficiency

Immunodeficiency may be primary, where there is an intrinsic problem with immune system development or function, or secondary, where immune function is affected by other factors such as medications, chronic disease or medical treatments. Primary immunodeficiency disorders include a range of conditions affecting antibody production, cellular immunity or combined immune pathways. One of the more recognised conditions in this group is common variable immunodeficiency (CVID), although there are many other forms. Secondary immunodeficiency may occur in association with medications, chronic illness or other systemic conditions that affect immune function.

Where immunodeficiency is confirmed or strongly suspected, ongoing specialist follow-up is often required to monitor infection burden, immune function and treatment response over time.
